The NHS Health Check is a health check-up for adults in England aged 40-74. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. During the appointment you will find out ways to lower your risk.

To receive a health check, it is important that you meet the eligibility criteria stated below:

• Aged 40-74

• Resident of East Sussex and/or registered to a GP in East Sussex (not including Brighton and Hove)

• NOT taking medication for high blood pressure, high cholesterol, atrial fibrillation, diabetes or kidney disease

• Haven’t had an NHS Health Check in the past 5 years

• Haven't previously had a heart attack, stroke or TIA
